When an athlete is fully recovered from previous training and competitions and has good energy stores and is hydrated they can usually perform well at high heart rates for an extended period of time.

The cyclist above has a lactate threshold of around 170 - 175 beats so as you can see he is operating at a very high level for a long time. Note his comments as to his performance.

In contrast if an athlete is very fatigued and or poorly nourished  there will often be great dificulty in achieving and maintaining the heart rates in their optimum ranges.
